Every line of the plan shows the table or section it comes from. Where the guideline is silent, the tool says so and leaves it to the operator. It never fills a gap.
Local readings. These are the tool author’s interpretation (24 Oct 2026) and have not yet been confirmed by the guideline owner. For very low risk procedures, anticoagulation is not routinely stopped and no thresholds are applied. The chronic liver disease INR bands are read as <1.5, 1.6–2.4 and ≥2.5, with exactly 1.5 left to the operator. These lines are marked “local reading”.
